Destyni Blaylock


Ms. Blaylock is a Nurse Practitioner with 15 years in the medical field. Her career path started when she served in the Army for four years (2000 to 2004).  She attended the U.S Army Medical Center and School where she became an LPN. Following the military, Ms. Blaylock went on to attend Chamberlain College of Nursing where she obtained her Bachelor’s Degree and her Master’s Degree, finishing and becoming certified in 2017.

Her work experience includes over 10 years of experience with the VA,  interning at the Healing Hands Primary Care (Fall 2016) and Healthcare Clinic at Walgreens (Spring 2017).  These experiences have allowed her to perform health screenings and provide wellness services for patients of all ages.  Since then, Ms. Blaylock has worked in Jacksonville hospitals including: Memorial Hospital, Department of Veterans Affairs Outpatient Clinic, and Life Care Rehabilitation Center of Orange Park.   When not caring for patients, she loves reading novels and spending time with her family.  Destyni sees patients in our Jacksonville offices.
